Where are we located?
Our office is located inside ADAPT Performance Center located at 136 Stutts Rd Suite #4 Mooresville, NC 28117.

What are our hours?
Office hours are as follows:

Monday through Thursday - 9:00 am- 5:00pm.
Fridays - 9:00 am to 1:00pm.

PLEASE NOTE We can only treat patients who reside/are located in North Carolina OR South Carolina.

Do you accept insurance?
WE DO NOT ACCEPT ANY FORMS OF INSURANCE for Office Visits.* We can provide detailed receipts at your request that you can submit to your insurance provider to see if there is any coverage for out of network medical care. It is rare with this type of practice that office visits will be covered but will vary depending on the care you receive. Most HSA and FSA cards are accepted.

*If you have COMMERCIAL insurance we may be able to utilize this for labs and medications! We will always try to find the most cost effective way to keep you on top of your health! WE ARE OPTED OUT FOR MEDICARE AND MEDICAID FOR ANY LAB ORDERS OR MEDICATIONS - TRICARE IS NOT GUARANTEED TO PROCESS*
What will it cost to become a patient?
Initial evaluation cost is $199.00. This includes a 60 minute assessment of your concerns with the provider.
There is NO obligation for ongoing care and membership.

To become an active patient in our ongoing medication management program there is a monthly membership fee of $75.00 (Seventy-Five), starting the month following your initial evaluation (on either the 1st or 15th). There are no contracts. However, we do ask for a two week notice for cancellation to help coordinate care when needed. We want you to be a patient with us because YOU want to be, not because you've signed a contract.
Follow up visits are 30 minutes in length and are charged $50.00 (Fifty) per visit.
Frequency of these visits will be determined by you and your provider as you develop your individualized treatment plan. For example HRT can have as few as 3-4 follow ups per year after the initial evaluation. We encourage you to speak with your provider to determine the best plan and schedule for you.
On average, Testosterone Replacement services end up being approximately $110/mo when all fees, visits, labs, and medication costs are taken into account. Weight loss services can vary significantly depending on what your insurance will or will not cover, but please feel free to call and ask specifically about this!

How are we different from other clinics?
The main difference between our medical practice is TRULY individualized treatment. There are no contracts or annual fees. All medications are prescribed at cost and are discussed upfront with you. We will utilize insurance to get your lab work covered to the best of our ability. We will complete Prior Authorizations to try to get your medications covered by insurance when we can. The compounding pharmacies utilized can save money for some patients in the long run and they will ship medications directly to you. Sometimes it is cheaper to use your local pharmacy with your insurance or GoodRx coupons. We will always find the most cost-effective option for you.
How often are labs required?
This is variable per patient. If you have had labs done within the last 3 months these will certainly be honored as long as medications have not been changed since then that could change your laboratory data. Or if symptomology has changed new labs may be required. Typically, for HRT, labs are required at your initial appointment and again ~4-6 weeks later. Or after a change has been made, and every 3-6 months thereafter. It varies on the individual.
PLEASE READ - We do NOT draw labs in office however we are integrated with Lab Corp and send electronic orders directly there for you. You will present there with ID and Insurance Card. We do not schedule appointments for you. That would be your responsibility.
How will I receive my medications?
Medications will be sent electronically directly to the pharmacy of your choice available for you to pick up, or the compounding pharmacies we have a relationship with will ship your medication directly to you.

What’s the process of transferring from another clinic?
This is a very simple process. Sign a release of information and/or just acquire/request your most recent lab work/documentation to be forwarded to our office prior to your initial appointment if possible. Not required, but certainly helpful! Email to send lab work is info@ascendhealthwellness.com
